Please review the attached moisture survey as per TAS 126 performed by A. Tomassi Roof Testing, Certification 03-0317.03. I have found the following non-conformance:

1.    Mr. Tomassi is not qualified to perform calculations and sign the report, TAS 126 – 1.3.

2.    The type of NDT is not cited on the report, and there is no instrument certification, TAS 126 – 14.1.2.

3.    Some roof sections did not identify the decking, TAS 126 – 14.1.1.

4.    There is no field recording sheet, TAS 126 – 14.1.3

5.    No core sample test report is provided, TAS 126 – 14.1.4

6.    No analysis of data, histogram, and statistical analysis is provided.

 

The moisture map does not show rooftop structures, equipments, drains,

penetrations, precise locations of the core cuts. Some critical areas of ponded water were not indicated.

 

Based on this report the owner’s roofing contractor prepared an estimated that became part of its hurricane Wilma damage insurance claim.

 

I have come across to many cases like this and always look deep into the report itself and the credentials. However, other consultants and or adjusters may not be so aware and the insurance claim could be overpaid.

 

The faulty report may not reach the local Building Department and so this malpractice is never discovered.

 

Attached you will find the article “Storms hit Florida with double whammy” posted on Miami Herald on May 22, 2006. Whatever the insurance industry overpay a damage claim, the homeowner’s insurance premium increases accordingly.

 

Even though The Product Control Division has its Quality Assurance service to investigate cases as above mentioned, it will be deployed only when it is reported.

 

I would like to suggest you as well as to the Florida Building Commission, Mr. Mo Madani, to include in the proposed 2006 Supplement to FBC-2004 discussion, that will take place next month FBC meeting, the following article:

 

“Any report issued per a Product Control Division certified Lab, according any criteria set in the Test Protocols for High-Velocity Hurricane Zone shall remit copy to the division for quality control and archives or mandate that prior issuance the report receive a bless of the local jurisdiction”

 

The basic idea is to close monitor what these Labs’s are doing in order to promptly correct or adjust the system as well as to have a more uniform testing procedures, records and reports.

 

I could file complain if I had found the proper form. However, you can take this email and all its attachments as being one.
