Re:     FL2982-R3 (Affirmation)

The subject Florida Product Approval Application was applied for as an affirmation application. The following actions and further examination were taken.

 

1.      The application lacked a QA Contract date.  As we are doing with all affirmation applications on March 24, 2009 a request was made for a QA Contract in order to provide this information on the application.

2.      When a reply to our request for information on the QA Contract was not received, on April 2, 2009 a second request for the QA information was requested.  On this email request, the BCIS contact on the selected QA Agency was notified.

3.      There has not been response on the two requests to provide an effective QA Contract.

4.      Upon further review of the application the following was found:

a.  The application is for an aluminum product to be used as a soffit.

b.  There is no specific testing standard on the building code for aluminum soffits.

c.  Applicant used the “Test Report” method of application for a product where there is no specific test required on the building code.

d.  The indicated method for application of a product approval would have been evaluation report by FL PE/RA.

 

This is a request to discuss Florida Product Approval Application FL2982-R3 (Affirmation) based on the above described information.
